Adult Medicine

The Adult Medicine Department at the Sacramento County Health Center provides comprehensive primary care for adult patients, offering chronic disease management, preventive care, and coordinated services that support overall wellness. The department also maintains dedicated same‑day appointment capacity to ensure timely access for urgent needs, with same‑day scheduling

Family Medicine

The Family Medicine Department at the Sacramento County Health Center provides comprehensive, patient‑centered primary care for individuals and families, offering preventive services, chronic disease management, and coordinated care across the lifespan.

Pediatric Medicine

The Sacramento County Health Center, Pediatric Department provides comprehensive primary care for children as part of a federally qualified health center serving low‑income and medically underserved families across the county, offering services delivered by pediatricians, residents, and nurse practitioners. The department also includes the Circle Clinic, a specialized program providing trauma‑informed care for children and youth in foster care, making it one of the Health Center’s unique and essential services.

School-Based Health Centers

In a groundbreaking collaboration, Sacramento County is working with multiple school districts to provide on-campus mental health services and link students in underserved schools to essential medical care, including well-child visits and immunizations at their local FQHC or Health Centers.

Primary Care drop-in appointments at Friendship Park, to ensure that everyone has access to the care they deserve.

Mobile Medicine Van

Wellness Without Walls (W3), a funded partnership with Primary Health and the SHPU, is a mobile unit delivering clinical services – including HIV, sexual health, and broader services – and referrals to unhoused communities and transitional-aged youth. This mobile clinic aims to increase access to health services, information, and resources among marginalized communities in Sacramento County. This program is made possible with funding from the Health Resources and Services Administration (HRSA), and the Centers for Disease Control and Prevention.

The Sacramento County Health Center (SCHC) is a Federally Qualified Health Center committed to providing accessible, high‑quality primary care for Medi‑Cal members and uninsured or under‑insured residents of Sacramento County. Our compassionate care teams deliver comprehensive Adult Medicine, Pediatrics, and Family Medicine services designed to support the whole person. In addition to our main clinic at 4600 Broadway, SCHC brings care directly into the community through our Mobile Medicine Van, a service site at Loaves & Fishes, and multiple school‑based clinics focused on delivering vital mental health services to children in underserved neighborhoods. At every location, SCHC strives to reduce barriers, promote wellness, and ensure that every individual has the opportunity to receive the care they deserve.
