How it works:

1. Patients receive care through a Sacramento County DHS program.
2. Provider submits a prescription to County of Sacramento Primary Care Center Pharmacy. 
3. Prescriptions are managed and communicated through DHS MyChart (OCHIN).
4. Medications are coordinated through the Primary Care Center Pharmacy.

This approach helps streamline care and reduce confusion around prescriptions.

Eligibility and Cost

To use the Primary Care Center Pharmacy:

• You must be enrolled in a Sacramento County DHS program
• Eligibility depends on the County program you are part of

Medications are offered at a reduced cost for eligible patients.

The Primary Care Center Pharmacy provides services in multiple languages. Multilingual staff is available to help patients understand their medications and care instructions.
