Primary Care Pharmacy

Program Overview

The Primary Care Pharmacy helps patients enrolled in Sacramento County Department of Health Services (DHS) programs. The program works together with the doctor to help patients obtain affordable medications, understand their medications, and safely take them. 

What Is the Primary Care Pharmacy?

The Primary Care Center Pharmacy is County-operated. It serves people who are already receiving care through Sacramento County DHS programs and is not a walk-in or retail pharmacy open to the general public.

The pharmacy is part of the County health system; it can coordinate directly with providers and support patients throughout a patient's care.

Many patients choose the Primary Care Pharmacy because it makes managing medications easier.

  • Lower medication costs for many patients, including those with Medi-Cal
  • One place to receive medications connected to County care services
  • Direct coordination between pharmacists and County providers
  • Shared electronic health records, helping resolve prescription issues more quickly
  • Multilingual services available

How it works:

  1. Patients receive care through a Sacramento County DHS program.
  2. Provider submits a prescription to County of Sacramento Primary Care Center Pharmacy. 
  3. Prescriptions are managed and communicated through DHS MyChart (OCHIN).
  4. Medications are coordinated through the Primary Care Center Pharmacy.

This approach helps streamline care and reduce confusion around prescriptions.

Accessing MyChart

Health Center patients can access personal health information online through MyChart.

Eligibility and Cost

To use the Primary Care Center Pharmacy:

  • You must be enrolled in a Sacramento County DHS program
  • Eligibility depends on the County program you are part of

Medications are offered at a reduced cost for eligible patients. 

 

Language Access

The Primary Care Pharmacy provides services in multiple languages. Multilingual staff are available to help patients understand their medications and care instructions.

Language assistance is available upon request.

If you are currently receiving care through a County DHS program:

  • Talk with your provider about using the Primary Care Pharmacy
  • Sign up for DHS MyChart (OCHIN) to stay connected and manage prescription refills

If you are not enrolled in a County program, your provider can help determine what services may be available to you.
