Please note:

After submitting a request, a program team member will contact you within three business days to follow up. Requests are processed Monday through Friday during business hours. Services are not immediate and depend on screening and availability. 

Open assessment hours may also be available through community providers.

  • Language and cultural needs are considered during screening 
  • ADA accommodations are available upon request
